mcp changes

Each MCP publish that changes a tool is a numbered release. This lists them, newest first, with what happened to each tool: added, renamed, removed or changed, with the inputs it gained (+, with * when required), lost (-) or changed (~). Breaking changes are marked. The heading says whether Tell agents what changed is on. See change notices.
